出生医学书面证明材料如何办理已经从国内学校毕业，户口已经不在学校，转到某单位上去了，可是突然要这个东西，怎么办理？是否仍然可以在学校办？还是要到户口所在地办理？有哪些程序？有没有通用的格式？是否需要中英文对照？大虾们知道多少请告诉我多少！谢谢！！！
还有，不知道这个本人的出生医学证明要用在哪里？我已经订好了机票没几天就要过来了，突然接到信说办理EP要这个：

1.EP in-principle approval duly signed by you
2.Your passport
3.The white immigration card (this will be issued to you upon your arrival in Singapore)
4.Pre-employment medical form and chest x-ray & HIV reports
5.1 recent passport-sized photograph of yourself
6.Educational certificates (Bachelor, Masters, PhD with English translation, if applicable) / professional memberships
7.Your birth certificate (if applicable)
8.Your marriage certificate (if applicable)
9.The birth certificate of your child / children (if applicable)

你说的是birth certificate？这个就是出生公证啊。拿着你的户口本和身份证到当地的公证处就可以办了。
